🚨Total Triage🚨

From 01.10.2025, as part of the Total Triage system, we are implementing this process partially for all query requests. As part of this transition:

Ø Appointments, prescription queries, referrals, sick notes, letter requests/queries and administrative queries can be submitted via the NHS website/Accurx.

Please Note: We will review your request on same day (Monday–Friday, 08:00–18:30). You will receive a message from the practice confirming how your request will be handled. This does not mean you will be dealt with/seen on the same day, however you will be informed of next steps the practice is taking to deal with your query/request.

We appreciate your co-operation in completing the form carefully and accurately. Incomplete or inaccurate information may lead to delays in processing your request.
